CLA APPLICATION CHECKLIST

The following items must be completed and submitted to Creative Learning Academy for your child’s application to the school.

1.    Application form. (Preprimary through 8th grades)

2.    Student Questionnaire. (5th through 8th grades)

3.    Record Release form.  To be signed by parent and given to current school along with teacher recommendations.

4.    Teacher recommendation forms.  Must be completed by current teachers and mailed by teachers to:

For K through 4th grades:  1 teacher recommendation
For 5th though 8th grades:  2 teacher recommendations

5.   Copies of most recent report card, last 2 years' report cards and 3 most recent standardized test scores. (Should be sent to CLA by current school.)

6.   An application/assessment fee of $100.

7.  An assessment (completed during classroom visit for 1st through 8th grade applicants). The Gesell Developmental screening will be administered to all new entering Preprimary and Kindergarten students.

Each student applying to Creative Learning Academy is required to visit in
his or her current grade level classroom.

•    Applying to Preprimary:  2 hour visit from 8:30 a.m. – 10:30 a.m.
•    Applying to Kindergarten:  2 hour visit from 8:30 a.m. – 10:30 a.m.
•    Applying to 1st – 8th grades:  1 full day visit:  8:00 a.m. – 3:00 p.m.

After the visit, the student and parent(s) are required to meet with the Head of CLA, and/or Director of Admissions.
